Who Is Eligible?
- Must be a resident of California
- Must be 1 to 19 years of age
- Must be a U.S. citizen or eligible qualified alien
- Family income must be within guidelines (ask your assistor for details)

please call our offices toll free at 1-866-535-8312 and an assistor will be
happy to answer any questions.
Once you qualify and your application is complete,
you can get coverage in as little as 10 days. (Other programs also available!)
The following is a list of items you will need to help you enroll quickly:
- Birth certificate and/or proof of legal residency
- Social Security card
- Paycheck stub or tax return (most recent)
- Identification of mother/father
